Emergent issues are addressed.<br \/>\n\u2022 Examination and building of medical, psychological, social, behavioral, vocational, recreational\/leisure, and developmental aspects of the resident\u2019s situation and reflects the need for inpatient psychiatric care on an ongoing basis.<br \/>\n\u2022 Medical services include regular health monitoring by medical staff and prompts, and ongoing follow up of any identified problem especially with emergent issues.<br \/>\n\u2022 Addressing the needs of special populations: In the evaluations and orientation. Staff will identify special needs. (ex. translators, readers, referrals for additional services, etc.)<br \/>\n\u2022 Active treatment planning development within 72 hours of admission and ensuring the exchange of information regarding the person\u2019s emergent problem.<br \/>\n\u2022 Treatment meetings<br \/>\n\u2022 Provide an integrated program of therapeutic, activities, and experiences to meet objectives of the Plan of Care (POC).<br \/>\n\u2022 Assessing the recipient\u2019s immediate and long-range therapeutic needs developmental priorities and personal strengths and liabilities in accord with the POC development requirements.<br \/>\n\u2022 Assessing the potential resources of the recipient\u2019s family.<br \/>\n\u2022 Setting treatment objectives in accord with the requirements above.<br \/>\n\u2022 Prescribing therapeutic modalities to achieve the plan\u2019s objectives.<br \/>\n\u2022 Crisis assessment.<br \/>\n\u2022 Decisions concerning persons served.<br \/>\n\u2022 Review the Plan of Care as needed or at a minimum of every thirty days by the facility treatment team to:<br \/>\n\u2022 Education on wellness and recovery<br \/>\n\u2022 Families are encouraged to participate.<br \/>\n\u2022 Development of a social support network<br \/>\n\u2022 Education\/training in selecting and maintain housing that is safe, decent, affordable, and accessible.<br \/>\n\u2022 Development of community living skills<br \/>\n\u2022 Involvement in community activities<br \/>\no Determine that services being provided are or were required on an inpatient basis.<br \/>\no Recommend changes in the plan, as indicated by the recipient\u2019s overall adjustment as an inpatient.<\/p>\n<p>\u2022 Advocacy: Addressing self-advocacy activities with the residents served.<br \/>\n\u2022 Contingency planning<br \/>\n\u2022 Future planning<br \/>\n\u2022 Addressing ongoing issues during stay at PRTF<\/p>\n<p>Note: Each medical evaluation will include<br \/>\na. Diagnoses<br \/>\nb. Summary of medical finding<br \/>\nc. Medical Behaviors<br \/>\nd. Mental and physical functional capacity<br \/>\ne. Prognosis; and<br \/>\nf. Physician\u2019s recommendations<\/p>\n<p>Referrals from New Way: Staff will engage that residents in resource management by<br \/>\nidentifying valuable community resources and connecting and referring the residents to<br \/>\nthose resources, including church services, specialty medical services, community<br \/>\nrelated recreational services, hob related services, educational services, etc.<\/p>\n<p>Scope of Services: The scope of services will be shared with the residents\/families or<br \/>\nother supporting individuals at the time of orientation. Those in addition to the resident<br \/>\nwill receive such information in accordance of the choices of the resident. and as a<br \/>\nminor, is the choices of that resident\u2019s legal guardian. Referral sources will receive a<br \/>\ndescription of the scope of services through the brochures and through conversations<br \/>\nwith office staff and these marketing New Way during recruiting processes verbally.<br \/>\nPayers and funding sources will be exposed to the scope of the services by accessing<br \/>\nspecific policies and procedures relevant. Finally, general public will receive information<br \/>\non the scope of services through brochures and by calling our office for more information<br \/>\non our services. When feasible, staff members will sit on community boards and will<br \/>\nmake other community presentations to inform the community of the scope of services.<br \/>\nThe Residential Treatment Policy, which describes the scope of services, will be<br \/>\nreviewed at least annually by Co.-CEOs along with all other policies and procedures,<br \/>\nand updates will be made as deemed necessary.<\/p>\n<p>Resources: New Way\u2019s PRTF Division is on\u00a0 a 9.5-acre property. \u00a0The PRTF has ample materials for staff to use with residents, including a number of skills training and counseling text\/work books. We have central air conditioning, a fax machine, copiers\/printers, and computers available. We keep<br \/>\nadequate supplies for hygiene needs as well as office supplies. The MHR building<br \/>\nconsists of several offices used by PRTF counselors for face to face<br \/>\ncounseling. The PRTF has a lobby area for a relaxed counseling setting. There is<br \/>\na conference table and chapel that is available for meetings or group activities. Each<br \/>\nresident has adequate space in his room\/dining room\/living area to study. Furniture is<br \/>\nappropriate size with sofas and chairs for all residents 6-17 years old, and parents,<br \/>\ncomfortable for all. There is also a basketball goal, reading materials, games, and<br \/>\nvideos.<\/p>\n<p>New Way maintains our exterior areas that are accessible to the clients, including the<br \/>\ngrounds and structures on the grounds, in good repair and free from potential hazards to<br \/>\nhealth or safety.<\/p>\n<p>Service Delivery Models\/Strategies<br \/>\nNew Way PRTF uses a variety of texts to ensure that residents and staff have adequate<br \/>\nresources, including but not limited to:<\/p>\n<p>a) Teaching Social Skills to Youth<br \/>\nb) The 1,2,3s of Treatment Planning<br \/>\nc) The CARE Model<br \/>\nd) Pathways workbook<\/p>\n<p>New way implements Cognitive Behavioral therapy and Eclectic Therapy. We use a<br \/>\nvariety of information pulled from websites through the Family Systems therapy. Department of Health and Hospitals, and evidenced based practice sites such as SAMPSA. We also research<br \/>\ninformation for mental health training through a variety electronic source. We do<br \/>\nwhatever is necessary to meet client\u2019s needs. Finally, we have a variety of functional<br \/>\nassessments. The Co-CEOs have researched and have co-authored multiple articles<br \/>\non mental health related issues. New Way shall provide written materials to residents<br \/>\nat age-appropriate levels to commensurate their education.<\/p>\n<p>Treatment of Minor Clients as of 6-17<br \/>\nBecause residents served may not have the capacity to make his\/her own decisions or<br \/>\nhave in his net interests, the DCSF\/OJJ case, the parent\/guardian with legal<br \/>\ncustody will be on file and will be assigned to make decisions for health care choices,<br \/>\nfinancial decisions, or life care planning. This issue will be discussed with residents and<br \/>\nthat consenting authority. New Way welcomes advocates and residents assigned or<br \/>\nother attorneys to participate in the decision making.<\/p>\n<p>Credentials Needed for Treatment Modality<br \/>\nThe Licensed Professional Counselor or a masters\u2019 level mental health professional<br \/>\nuses Cognitive Behavioral therapy, Family System Therapy, or Eclectic Therapy and may use the 1, 2, 3\u2019s of Treatment Planning or the Pathways workbook. The RN, LPN, or tech use the Teaching Social Skills to Youth, Casey Life Skills Training or The CARE Model.<\/p>\n<p>Mechanisms to Address the Needs of Special Populations<br \/>\nNew Way of Southwest Louisiana LLC Outpatient Treatment Program addresses the<br \/>\nspecial needs of the clients served through the development and ongoing monitoring<br \/>\nand modification of the individual plan. Through this process, the strengths, abilities,<br \/>\nneeds, preferences and desired outcomes will be developed based on the unique<br \/>\nqualities of the clients served. Specific accommodations to address special needs may<br \/>\ninclude the following amplification devices and writing boards for use with hearing<br \/>\nimpaired clients, magnification sheets for the visually impaired, and language<br \/>\ninterpretation of non-English speaking clients.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>\u2022 Diagnostic evaluation within the first 24 hours of admission.
